Application of prosthetic adhesives remains the conventional method and a very reliable and secure means of attaching orbital prostheses. We employ the best adhesives and placement strategies based on 35 years of experience specifically in this specialty.

When patients have very delicate skin or adhesive sensitivities or prefer the simplest way to wear their prosthesis without surgery we simply attach the orbital prosthesis to attractive eyeglasses.

This patient has received a hollow extremely lightweight orbital prosthesis that slips in and stays secure requiring no adhesive, no bone-anchored implants and no other mechanical means of retention.
This technique was published in a chapter written by Greg Gion, MMS, CCA on orbital prostheses in the textbook Facial and somatic Prosthetics (McKinstry, 1995). The technique was also included in a chapter on orbital prosthetics in Principles and Practice in Ophthalmic Plastic and Reconstructive Surgery (Bosniak, 1995).
Of critical importance is that the bulb portion of the prosthesis seat absolutely passively in a very accurately captured exenteration cavity. The bulb walls are generally less than 2 mm in thickness with a mid-low durometer silicone.
The technique was painstakingly developed to allow the elderly patient to enjoy their prosthesis without adhesive or fear of tissue trauma.

Our client received a 2-piece magnetic prosthesis which positions into the cavity in 2 steps. First, a soft silicone insert with a central neodymium magnet fits intimately into the skin-lined cavity creating a gentle but secure attachment. Secondly the aesthetic component clicks into place magnetically.
It stays in place with no adhesive and the orbital cavity now stays cleaner and is easier to maintain.
Our experience allows us to exploit opportunities that we know will make the patient’s life a lot easier with the prosthesis.

The larger orbital prosthesis is sometimes classified as an upper facial prosthesis or hemifacial prosthesis. Orbital exenteration with hemimaxillectomy is a very ablative surgery that necessities a larger facial prosthesis.
However, the prosthesis is designed very light in weight with a magnetic connector so that it may attach to the intraoral dental device called the obturator. Our design allows easy placement of the orbital prosthesis.
Both the intraoral and extraoral components snap together with specialty titanium encased corrosion-proof magnets we have identified for this purpose. The two prosthetic components thereby stabilize each other.
Taking this intensive approach we turn the devastating effect of the surgical deficit into an advantage for the patient.

The patient elected to have 3 osseointegrated implant fixtures placed in her orbital cavity so she could have a magnetic orbital prosthesis. In most cases this process is just like dental implants. You go home the same day after the surgery and in a few months come back for the final prosthesis.
It is a wonderful option just letting the magnetic components hold the prosthesis securely in place with no adhesive at all.

The Prosthetic Eye Process
Creating your custom eye prosthesis and orbital prosthesis is a hands-on, collaborative process guided by our expert team. We can complete many types of eye and orbital prostheses in as few as 3 days:
We first review your doctor’s referral notes for your prosthesis and discuss your goals for wearing and caring for your prosthesis. We make sure the affected area is well healed and ready for prosthetic restoration.
We gently apply a special silicone to record your orbital area contours. It sets in just a few minutes then lifts off cleanly with ease. We also use the same material to check the fit as seen here.
In shaping your orbital form we often find that critical eye gaze/positioning and direct wax sculpting of lid nuances develop much more quickly and accurately than when using our scanning/3D printing tools.
We watch your eye shape change when you smile and decide what eye expression works best for you.
Your skin colors and character details are all captured using each anaplastologist’s unique reference color swatches and/or silicone formulations mixed during your session.
In our labs we spend hours refining the wax to include every detail in your eyelids and surrounding skin texture. Every wax detail will be recorded in the investment process.
Your skin tones and characteristic details like tiny blood vessels, freckles, moles, etc. are carefully mixed in silicone then painted into the mold.
This method of placement of silicone into the mold, also known as intrinsic coloration, demands patience and experience for your best orbital prosthesis aesthetics.
Your demolded silicone prosthesis will be trimmed and prepared with the custom artificial eye created by your anaplastologist or ocularist.
In this case, the beautiful ocular prosthesis was created by Strauss Eye Prosthetics in Rochester, New York.
Your anaplastologist will then instill all the final characterization details using extrinsic tints which are sealed in and imparted with skin texturing.
Finally, artificial eyelashes are painstakingly inserted into the eyelids.
This adhesive-retained orbital prosthesis was successful. Our client later decided to have the very user-friendly glasses-retained orbital prosthesis (see link).

Frequently Asked Questions
We can complete many types of eye and orbital prostheses in 3 days if you want to come and stay near our office. If we select a more complex attachment system, it might require just an extra day or two.
Otherwise, you can arrange to make separate visits. We are very flexible and will create a plan that works for you. (The magnetic types that attach to bone-anchored posts take longer because of osseointegration time which is 2-3 months)
No. A lifelike silicone prosthesis is not implanted under your skin or attached to your body permanently. Even if it is a bone-anchored (osseointegrated) prosthesis, it must be taken off regularly to be washed and to maintain the cleanliness and health of your skin.
Orbital/eye prosthetics may be retained with adhesive strategies we employ for each client, or adhesive-free methods including anatomical fit, magnetic attachment to abutments or dental devices, or by attaching the prosthesis to glasses.
We go over all the methods that will work best for you, including osseointegration. Your doctor will help determine if osseointegration is appropriate.
Your preference counts the most, but we make the decision together.
Osseointegration is when small screws fixated into bone act as permanent anchors for the prosthesis.
Surgeons we collaborate with place the fixtures, then we use the magnetic components that attach the prosthesis to the fixture posts (abutments).
We have been refining osseointegration attachment to orbital, nasal, and auricular prostheses since 1984.
In general, we recommend that you remove your prosthesis at night for cleaning and to allow your skin to breathe.
Some patients then learn from experience that they can adjust their wearing schedule. We provide personalized wear and care instructions for each client.
Almost all silicone prostheses are washed daily using isopropyl alcohol, detergent soaps like Dawn or Dove, and then rinsed, and kept dry when not in use.
However, each prosthesis is unique and might require special precautions. Ultimately, we will walk you through your daily care plan and provide specific instructions tailored to your unique prosthesis.
The cost of a silicone orbital prosthesis that includes the ocular or artificial eye component will depend on your specific condition and the wide range of attachment options appropriate for you, including osseointegration.
It is possible that your insurance will cover some or all the costs. After we receive your permission and insurance information our insurance specialist will help determine the level of coverage of your insurance plan.
The medical and insurance community often say 2 or 3 years is the average lifespan of a silicone facial prosthesis. Insurance might then pay for a replacement.
Many of our patients wear the original prosthesis for several more years with an occasional retinting. Identical replacement prostheses are then available from the original mold taking just a day or two and at substantially less cost to you.
We do not recommend wearing your prosthesis during contact sports, or vigorous activities where it could be dislodged and lost. Otherwise, use your prosthesis however you feel comfortable.
These activities will not hurt the prosthesis. However, vigorous movements and/or sweating could affect the bond/security of adhesive-retained prostheses.
We recommend wearing your prosthesis at home at first during strenuous type activities to become familiar with the strength of the prosthesis attachment.
We do not recommend sleeping with the prosthesis on – at least not on a regular basis.
This is for maximum longevity of your prosthesis’ surface finish and for the health and hygiene of your skin under the prosthesis, especially for implant-retained orbital prostheses.
