Medical Art Prosthetics Forms
If you would like to get a head start on your new patient paperwork, you can complete the Patient Registration Form below. Please try to complete as much as possible, but do not worry if you cannot fill in all the blanks. We will review the forms with you during your consultation.
Prosthetics Information Sheets
Literature in the field of anaplastology is rare, so please download and print these handouts for your future reference. We do not aggressively market our specialty, but we want to educate and remain visible so we can be found when the need for a quality aesthetic prosthesis arises.

Are you traveling from out-of-state or from another country?
Coming to our office for your prosthesis is easy, whether you are traveling from another state in the USA or from Japan or South Africa, as some of our clients have. It usually only requires you to stay in a nearby hotel in Madison for 3-5 days so we can work with you an hour or two each day.

Here is a typical plan for you to receive your facial prosthesis or your partial hand or foot prosthesis in one trip:
- Day 1 (usually Monday): We meet and get to know each other, and then start the process by taking simple impressions of the affected area. After lunch we might have a design/sculpting session and you can already see your prosthesis taking shape.
- Day 2: We meet in the morning to finish sculpting or double check fit or any issues you want to address. We also take photos of color references and our wax prototypes in place, then dismiss for the day.
- Day 3: The patient might be free much of the day or the entire day to explore the nice college town of Madison or nearby sites while we create your unique mold and prosthesis using all our reference materials.
- Day 4: The prosthesis is placed on you for final tinting and characterization, so it matches your skin precisely. The day is focused on getting everything just right. We demonstrate the proper use and care in handling your prosthesis right along with you, and we provide all the best adhesives and after care products, instructions and resources for you to take home.
Simple nasal or ear prostheses or single finger or toe prostheses can be created in just 2 or 3 days. Complex, orbital, hand and foot prostheses require 4 days or more.
A single photo of the area of concern will usually allow us to say exactly what we will do and how many days it will take. If CT scans were taken in the course of your treatment we like to use them! They are very helpful because we have digital design and 3D printing capabilities that allow us to create precision prostheses without your presence in our facility! Email a photo now and we will give you a very accurate treatment plan and costs.
